Apicoectomy (Endodontic Surgery)

An apicoectomy is a surgical procedure conducted when a root canal has not successfully resolved an infection. If an infection persists or recurs after the initial treatment, an apicoectomy may be necessary. During a root canal, it can be challenging to eliminate all infected tissue, as multiple nerves may harbor the infection. This procedure involves the removal of the tip of the tooth’s root, which is then filled to prevent further issues.
 
Typically, a second root canal is considered before opting for an apicoectomy, as it is a less invasive option. Prior to the surgery, a local anesthetic is administered to numb the area. The dentist will begin by making an incision in the gum tissue to access the root of the affected tooth. Any inflamed or infected tissue will be carefully removed to clean the area. Given the limited space involved, only a few millimeters of the root are excised, necessitating the use of magnification and fine instruments to ensure precision.
 
This meticulous approach contributes to a high success rate for the procedure. Once the root tip is extracted, a filling material is placed in the root canal, and the gums are sutured closed. Depending on the type of sutures used, you may need a follow-up appointment for removal, or dissolvable sutures may be employed. Over the following months, the bone will gradually heal around the treated root, restoring the health of the tooth.
